Medication Administration Policy

Objective: Liberty Classical Academy is committed to the safety of all our students and will follow strict guidelines, in accordance with Minnesota Law, when administering medication at school.

Scope: All students in grades K through 12

Definitions:
Prescription medication: Any medication requiring a physician’s prescription.
Non-prescription medication: Any medication available over the counter.

GUIDELINES

Authorized school personnel will dispense all medication, both prescription and non-prescription, from the school office.

Prescription Medications (see office for proper forms):

• Medication must be in the original pharmacy-issued container with the student’s name and prescription details on the label.
• Parents/Guardians must bring all medications to the school office with a physician-completed authorization form, specifying the name of the medication, amount and time(s) to be given and reason for its use.
• Medication will only be administered according to the directions on the prescription’s label or with a doctor’s written prescription.
• Any student requiring medication for asthma or respiratory disorders must have a physician’s treatment plan on file with the school office.
• Students may assume responsibility for maintaining and administering their own medication for disorders similar to asthma with request from the parents and physician and approval of the school office.
NON-PRESCRIPTION MEDICATIONS (SEE OFFICE FOR PROPER FORMS):
• Medication must be in the original container labeled with the student’s name and correct dose. • Medication must be kept in the nurse’s office, not in a student’s locker or backpack.
• An over-the-counter medication permission slip must be submitted to the office at the beginning of each school year for each student in order for them to receive over-the-counter medications.
• Medication will only be administered in accordance with the directions printed on package or with a doctor’s written instruction.
UNDER NO CIRCUMSTANCES WILL MEDICATION BE ADMINISTERED OUTSIDE THE ABOVE GUIDELINES.
In the case of a medical emergency while at school, emergency services will be contacted immediately, followed by the student’s parent(s).
